The first 5-6 weeks post-discharge involve frequent rehabilitation and mobility training. In-home nursing care supports medication adherence, fall prevention, and strength recovery to accelerate progress.

Functional Reintegration (3-6 Months) – Restoring Independence

Significant improvements happen in this phase. Patients focus on regaining functional skills, adaptive mobility, and cognitive recovery while adjusting to lifestyle changes.

Recovery slows, but long-term health monitoring, symptom management, and caregiver education are key to maintaining progress and preventing complications.

Stage 1: Acute Onset & Hospital Discharge (First 4 Weeks)

Goal:  Stabilization & Prevention of Early Complications

After a stroke or neurological event, early care is critical for preventing complications and supporting recovery. During this phase, our nurses help monitor vital signs, assist with safe mobility, and educate families on care techniques.
